About the Position
As a Care Manager, you will develop, implement, and monitor person-centered plans, utilizing your strong relational and organizational skills. Our care managers come from diverse backgrounds but are all experts in delivering top-tier training and support. We'll support your passion for service in a dynamic, hybrid work environment combining home visits, office meetings, and remote work. 🚗💻
If supporting members outside of New York City, a driver’s license, reliable transportation, and appropriate insurance are required.

Key Responsibilities
- Advocate for members: Ensure informed decision-making, consent, and guardianship support.
- Coordinate meetings & assessments: Lead interdisciplinary team collaborations to develop comprehensive, person-centered Life Plans aligning with members' needs and goals.
- Implement & monitor Plans: Update and facilitate reviews, ensuring plans reflect current needs and preferences.
- Support integration of services: Facilitate medical, behavioral, social, habilitation, dental, psychosocial, community, and long-term supports.
- Maintain contact: Keep ongoing relationships with key individuals in members’ lives.
- Documentation: Ensure timely submission of all required documentation, including Life Plans and progress notes.
- Benefits management: Assist members in maintaining benefits such as Social Security, Medicaid, Medicare, and Food Stamps.
- Problem-solving: Support members in resolving issues related to housing, utilities, safety, and legal matters.
- Report concerns: Immediately report any observed or reported abuse or neglect.

Qualifications
- Educational & Experience Requirements:
- Bachelor’s degree + 2+ years relevant experience, or
- Registered Nurse license + 2+ years relevant experience, or
- Master’s degree + 1+ year relevant experience.
- Skills: Strong verbal & written communication skills, along with excellent interpersonal and organizational abilities.

Compensation
Hourly Rate: $27.75 – $30.19/hour
