Provident Behavioral Health Is Hiring A CARE MANAGER (BILINGUAL IN HEBREW OR YIDDISH PREFERRED)

Position Summary: The Care Manager oversees and coordinates access to services for individuals with intellectual and developmental disabilities, developing and monitoring person-centered Life Plans. Responsibilities include conducting assessments, advocating for members, and ensuring access to healthcare and community services.

Duties & Responsibilities

• Deliver person-centered care management services.
• Complete comprehensive assessments and develop Life Plans.
• Advocate for members' needs and access to services.
• Maintain eligibility for care management and benefits.
• Coordinate healthcare services and transitional care.
• Ensure compliance with documentation and privacy standards.

Qualifications: Bachelors degree or relevant experience; bilingual in Hebrew or Yiddish preferred. Strong communication and interpersonal skills required.

Work Environment: Remote position with fieldwork and travel. Must adhere to privacy standards.

Job Type: Full-time; Pay: $29.00 - $31.00 per hour.
