Care Design New York Care Manager

As a Care Manager at Care Design New York, you will empower individuals with intellectual and/or developmental disabilities to achieve their personal goals by developing, implementing, and monitoring person-centered Life Plans. Your dedication and organizational skills will help our members live their best lives. All our Care Managers receive top-tier training, regardless of their background, and we're looking for passionate individuals eager to serve our diverse community. 🌟

This role offers a hybrid work environment involving in-home visits, office trainings, meetings, and remote work. If supporting members outside of New York City, a driver's license, reliable transportation, and appropriate insurance are required.

Key Responsibilities

  • Advocate for members: Ensure informed decision-making, consent, and guardianship responsibilities.
  • Coordinate and lead meetings and assessments with members and interdisciplinary teams to develop comprehensive, person-centered Life Plans that align with individual needs and goals.
  • Implement & monitor: Update and facilitate reviews of Life Plans and oversee approval processes.
  • Support integration: Ensure access to needed services including medical, behavioral, social, habilitation, dental, psychosocial, community-based, and facility-based supports.
  • Maintain relationships: Keep ongoing contact with key people in members' lives.
  • Documentation: Ensure timely submission of all necessary documentation, such as Life Plans and progress notes.
  • Benefits management: Assist members with maintaining Social Security, Medicaid, Medicare, Food Stamps, and other benefits.
  • Problem resolution: Help members address issues related to housing, utilities, legal concerns, and safety.
  • Report concerns: Immediately report any observed or reported abuse or neglect.

Qualifications

  • Educational & experience requirements:
    • Bachelor’s degree with 2+ years of relevant experience, OR
    • Registered Nurse license with 2+ years of relevant experience, OR
    • Master’s degree with 1+ year of relevant experience.
  • Skills: Strong verbal and written communication, interpersonal abilities, and organizational skills are essential.
  • Bilingual proficiency: Fluency in Mandarin or Cantonese and English required.
