Salary Range: $29.87 - $31.93 per hour

Position Type: Full Time

Travel Percentage: Up to 75% 🚗

🎯 Position Summary

The Care Manager plays a vital role within our Care Management programs, including:

  • Health Home Care & Comprehensive Care Management
  • HCBS Basic Plan Support
  • State Paid Care Management services

This position may also support Willowbrook Class Members.

The primary responsibility is to oversee and coordinate access to services for individuals with intellectual and developmental disabilities. Working closely with members, their families and/or representatives, and providers, the Care Manager develops, implements, and monitors an integrated, person-centered Life Plan.

✨ The Life Plan serves as the foundation for service delivery, identifying the necessary medical, behavioral, community, social supports, and other services that enable members to live their healthiest and most meaningful lives.

A key aspect of this role is advocating for members to access the services needed to reach their personal goals and live a fulfilling life.

ACA/NY is a 501(c)(3) not-for-profit organization, designated as a Care Coordination Organization/Health Home (CCO/HH) by New York State. We are dedicated to supporting over 25,000 individuals across New York City, Long Island, and the Lower Hudson Valley, offering comprehensive care management and service coordination to people with Intellectual and Developmental Disabilities.

🛠️ Duties & Responsibilities

  • Deliver person-centered care management services compliant with regulatory standards and agency policies
  • Complete comprehensive assessments and reassessments
  • Identify service gaps and make appropriate referrals
  • Advocate for members to reach their goals and enhance quality of life 🌟
  • Develop, implement, and monitor Life Plans within required timeframes
  • Manage and resolve conflicts in the person-centered planning process
  • Ensure timely and accurate documentation and billing compliance
  • Maintain member eligibility through annual Level of Care (Re)Determination & HCBS enrollment
  • Access and maintain benefits and entitlements (Medicaid, Social Security, SNAP, etc.)
  • Ensure updated and valid information sharing consents in health records
  • Coordinate access to medical, behavioral health, and specialized services
  • Monitor healthcare needs and provide follow-up on critical cases
  • Facilitate transitional care from inpatient to community settings
  • Utilize health IT tools, including electronic health records and telehealth services 💻
  • Participate in team meetings, trainings, and supervisions
  • Travel as needed within the service area to meet members and providers
  • Follow incident reporting protocols ensuring member safety
  • Maintain confidentiality per HIPAA standards 🔒
  • Uphold ACA/NY’s Code of Conduct and policies
  • Perform other duties as assigned

🎓 Qualifications

  • Bachelor’s degree (Arts or Science) with two years of relevant experience, or licensed Registered Nurse with two years of experience, or a Master’s degree with one year of relevant experience
  • Fluent in Arabic and familiar with community cultural practices and norms
  • Strong sense of integrity and a personal commitment to serving others ❤️
