Position Summary
The Care Manager operates within the Care Management programs, supporting initiatives such as Health Home Care Comprehensive Care Management, HCBS Basic Plan Support, and State Paid Care Management services. This role may also provide support to Willowbrook Class Members.
The core responsibility of this role is to oversee and coordinate access to services for individuals with disabilities, collaborating closely with members, their families and/or representatives, and service providers. The Care Manager develops, implements, and monitors an integrated, person-centered Life Plan based on comprehensive assessments. This plan serves as the foundation for delivering tailored services to meet medical, behavioral health, social, and community needs. 🧩
A key aspect of this role is advocating fiercely for members, enabling them to access services necessary to achieve their personal goals and lead meaningful, high-quality lives.

ACA/NY is a 501(c)(3) not-for-profit organization designated as a Care Coordination Organization/Health Home by New York State. We are dedicated to serving over 25,000 individuals across NYC, Long Island, and the Lower Hudson Valley by providing comprehensive care management and coordination of services. 💙
Duties & Responsibilities
- Deliver person-centered care management services in compliance with all regulatory standards, agency policies, and quality management plans.
- Complete comprehensive assessments/reassessments for members.
- Identify service gaps and make appropriate referrals.
- Advocate for members to reach their goals and enhance quality of life.
- Develop, implement, and monitor Life Plans within required timeframes, leading interdisciplinary team planning with the member at the center.
- Address and resolve conflicts in person-centered planning in collaboration with the team.
- Ensure all service documentation is completed accurately and timely, including billing documentation validity.
- Maintain member eligibility through annual Level of Care (Re)Determinations, OPWDD eligibility, and HCBS waiver enrollment.
- Identify and access benefits and entitlements such as Medicaid, Social Security, and SNAP, ensuring ongoing benefits.
- Manage and update information sharing consent in the electronic health record.
- Coordinate and provide access to healthcare services, including medical, behavioral health, and specialized services, with ongoing communication and follow-up.
- Identify and facilitate preventive and health promotion services.
- Coordinate transitional care, including follow-up from inpatient settings, discharge planning, and transfers within healthcare and residential settings.
- Utilize health information technology (e.g., EHR, telehealth) effectively to support care delivery.
- Maintain detailed, accurate electronic health records for all assigned members.
- Participate in team meetings, trainings, and supervisions.
- Complete all training requirements within designated timeframes.
- Travel within the service area to meet members, providers, and interdisciplinary team members, as necessary.
- Follow incident reporting protocols promptly to ensure member safety.
- Uphold confidentiality per HIPAA and privacy policies.
- Adhere to all policies and standard operating procedures.
- Uphold ACA/NY's Code of Conduct.
- Perform other duties as assigned to support the agency’s mission.
Qualifications
- Bachelor of Arts or Science degree with two years of relevant experience; or
License as a Registered Nurse with two years of experience; or
Master’s degree with one year of relevant experience. - Fluent understanding of community cultural traditions and practices of members and their families.
- Strong integrity and dedication to serving people with I/DD and their families.
- Excellent interpersonal, public speaking, and written communication skills.
- Ability to work autonomously.
- Demonstrates professionalism and respect, with the ability to collaborate effectively within a team.
Work Environment
