We are seeking a dedicated Bilingual Care Manager to provide person-centered care for individuals with intellectual and developmental disabilities in the vibrant New York area.
Position Summary
The Care Manager plays a vital role within our Care Management programs, including Health Home Care Comprehensive Care Management, HCBS Basic Plan Support, and State Paid Care Management services. Their primary responsibility is to oversee and coordinate access to a variety of services for members, working closely with individuals, families, and providers.
Through comprehensive assessments, the Care Manager develops, implements, and monitors Health and Life Plans that are personalized, goal-oriented, and support each member's wellbeing. They act as a strong advocate to help members access necessary services—helping them live their healthiest, most meaningful lives.
ACA/NY is a 501(c)(3) nonprofit organization, recognized as a Care Coordination Organization/Health Home by New York State, supporting over 25,000 individuals across New York City, Long Island, and the Lower Hudson Valley.
Duties & Responsibilities
- Provide person-centered care management following regulatory standards, agency policies, and quality management procedures.
- Complete comprehensive assessments/reassessments to evaluate member needs.
- Identify service gaps and make appropriate referrals; advocate to help members achieve their goals.
- Develop, implement, and monitor personalized Life Plans within established timeframes, leading interdisciplinary team planning processes centered on the individual.
- Address conflicts or disagreements within the planning process, working collaboratively to find timely resolutions.
- Ensure accurate documentation of all services, billing, and compliance requirements, maintaining timely submissions.
- Maintain member eligibility by completing annual Level of Care determinations, ensuring OPWDD eligibility, and managing HCBS waiver enrollments.
- Access and coordinate benefits such as Medicaid, Social Security, and SNAP, ensuring ongoing entitlement maintenance.
- Manage information sharing consent to keep health records current, updating as necessary.
- Coordinate healthcare services across medical, behavioral health, and specialized providers, with regular follow-up and monitoring.
- Facilitate preventative and health promotion services as needed to support overall wellbeing.
- Oversee transitional care, including discharge planning and facilitating service transfers from inpatient or residential settings, especially during the transition to adult services.
- Leverage health technology like electronic health records and telehealth tools to deliver effective care management and ensure comprehensive documentation.
- Participate in team meetings, trainings, and supervision sessions to stay aligned with agency practices.
- Complete required trainings within designated timeframes to maintain competency.
- Travel across the service area to meet members, collaborate with providers, and support interdisciplinary team activities, ensuring all needs are met efficiently.
