Position Summary 📝

The Care Manager provides services within various Care Management programs, including Health Home Care Comprehensive Care Management, HCBS Basic Plan Support, and State Paid Care Management services. This role may support Willowbrook Class Members.

The primary responsibility is to coordinate access to services for people with intellectual and developmental disabilities. The Care Manager collaborates with members, their families or representatives, and providers to develop, implement, and monitor an integrated, person-centered Life Plan. This plan forms the foundation for tailored service delivery, addressing medical, behavioral health, social supports, and other necessary services to promote a healthy and meaningful life.

Key to this role is being a strong advocate, supporting members in accessing the services they need to reach their goals and live a high-quality life.

We are committed to serving individuals with I/DD by providing comprehensive care management and coordination. Duties & Responsibilities 🛠️

  • Deliver person-centered care management services in line with regulatory standards, agency policies, and quality management plans.
  • Complete all assessments and re-assessments comprehensively and timely.
  • Identify service gaps and make appropriate referrals.
  • Advocate for members to achieve their goals and quality of life.
  • Develop, implement, and monitor Life Plans within required timeframes, leading an interdisciplinary team centered around the person.
  • Resolve conflicts or disagreements in planning processes with the team promptly.
  • Ensure all service documentation and billing prerequisites are completed accurately and timely.
  • Maintain member eligibility through annual Level of Care (Re)Determinations and manage eligibility for OPWDD and HCBS waivers.
  • Identify and facilitate access to benefits such as Medicaid, Social Security, SNAP, etc., ensuring ongoing eligibility.
  • Update and maintain appropriate information sharing consents in the electronic health record (EHR).
  • Coordinate access to high-quality healthcare and provide ongoing communication and follow-up for acute needs.
  • Facilitate preventive and health promotion services.
  • Manage transitional care, including discharge planning and transfers between healthcare settings, ensuring seamless service continuity.
  • Utilize health information technology, including EHRs and telehealth platforms, to enhance care management.
  • Participate in team meetings, training, and supervision as scheduled.
  • Travel across the service area to meet members and providers, as needed.
  • Follow incident reporting protocols to ensure safety and proper documentation.
  • Maintain confidentiality and adhere to HIPAA and privacy regulations.
  • Comply with all operational policies and best practices of ACA/NY and its code of conduct.
  • Perform other duties as assigned by management.

Qualifications 🎓

  • Possession of a Bachelor of Arts or Science degree with at least two years of relevant experience, or an RN license with two years of relevant experience, or a Master's degree with one year of relevant experience.
  • Fluent understanding of community cultural norms, traditions, and practices of members and their families.
  • Strong integrity and personal dedication to serving individuals with I/DD and their families.
