Salary Range: $29.87 - $31.93 Hourly

Position Summary 📝

The Care Manager plays a vital role in delivering services within diverse Care Management programs such as Health Home Care, Comprehensive Care Management, HCBS Basic Plan Support, and State Paid Care Management. This role may involve supporting Willowbrook Class Members.

Core responsibilities include overseeing and coordinating access to essential services for individuals with intellectual and developmental disabilities, working closely with members, families, and providers to develop personalized, person-centered Life Plans that promote health, independence, and quality of life.

The Care Manager is an advocate, ensuring members access necessary services aligned with their goals, helping them live their healthiest and most fulfilling lives.

About ACA/NY: A 501(c)(3) nonprofit organization designated as a Care Coordination Organization/Health Home by New York State, supporting over 25,000 people across New York City, Long Island, and the Lower Hudson Valley with comprehensive care management.

Primary Duties & Responsibilities 🎯

  • Deliver person-centered care management services adhering to regulatory standards, policies, and quality protocols.
  • Complete comprehensive assessments and reassessments for members.
  • Identify service gaps and make appropriate referrals.
  • Advocate on behalf of members to achieve their personal goals and enhance quality of life.
  • Develop, implement, and monitor individualized Life Plans within set timelines, leading interdisciplinary team planning.
  • Address conflicts in the person-centered planning process and collaborate with team members for resolution.
  • Ensure timely and accurate documentation for all services and billing processes.
  • Maintain member eligibility by completing annual Level of Care (Re)Determinations and managing HCBS waiver enrollment.
  • Access and maintain benefits and entitlements such as Medicaid, Social Security, and SNAP.
  • Update and maintain current consent for information sharing within electronic health records (EHR).
  • Coordinate access to healthcare services, including medical, behavioral, and specialized care.
  • Promote preventive and health-promotion services as needed.
  • Manage transitional care, including hospital discharges, follow-ups, and service transfers.
  • Utilize health information technology, including EHR and telehealth platforms.
  • Participate in team meetings, trainings, and supervision sessions as scheduled.
  • Travel within the service area to meet members and coordinate care as needed 🚗.
  • Follow incident reporting protocols to ensure safety and confidentiality.
  • Uphold HIPAA guidelines and organizational policies, including ACA/NY's Code of Conduct.
  • Perform additional duties as assigned.

Qualifications 🎓

  • Bachelor’s degree (BA/BS) with two years of relevant experience, or RN license with two years of relevant experience, or Master’s degree with one year of relevant experience.
  • Fluent in Mandarin or Cantonese, with a strong understanding of community cultural traditions, norms, and practices of members and their families.
  • High integrity and personal commitment to providing quality care.
