Position Summary

The Care Manager provides essential services within the Care Management programs, including:

  • Health Home Care Comprehensive Care Management
  • HCBS Basic Plan Support
  • State Paid Care Management services

This role may support Willowbrook Class Members.

The core responsibility of the Care Manager is to oversee and coordinate access to services for people with intellectual and developmental disabilities. 🤗

They work closely with members, their families or representatives, and providers to develop, implement, and monitor an individualized, person-centered Life Plan. This plan is the foundation of service delivery, identifying needs across medical, behavioral health, community, and social support domains to help members live their healthiest and most fulfilling lives.

As a strong advocate, the Care Manager supports members in accessing essential services to achieve their goals and live with dignity and quality. 💙

About ACA/NY

ACA/NY is a 501(c)(3) not-for-profit organization, designated as a Care Coordination Organization/Health Home (CCO/HH) by New York State. We are dedicated to supporting over 25,000 individuals across New York City, Long Island, and the Lower Hudson Valley with comprehensive care management and coordination services.

Key Duties & Responsibilities

  • Deliver person-centered care: Ensuring services meet regulatory standards and agency policies.
  • Complete assessments: Conduct comprehensive assessments and reassessments transparently and timely.
  • Identify service gaps: Make appropriate referrals and advocate for members to reach their goals.
  • Develop and monitor Life Plans: Lead interdisciplinary team planning centered on the individual.
  • Conflict resolution: Address and resolve disagreements effectively within planning processes.
  • Documentation & Billing: Complete all required documentation accurately and promptly, ensuring billing readiness.
  • Maintain eligibility: Conduct annual Level of Care (Re)Determinations, ensure OPWDD eligibility, and enroll in HCBS waivers.
  • Benefits & entitlements: Access and maintain Medicaid, Social Security, SNAP, and other benefits.
  • Information sharing: Keep current and accurate consent forms in the electronic health record (EHR).
  • Healthcare coordination: Facilitate access to medical, behavioral health, and specialized services, including follow-up and transition care.
  • Use of health technology: Utilize EHR systems and telehealth tools to deliver quality care.
  • Team participation: Attend meetings, trainings, and supervisions as scheduled.
  • Travel: Meet members and providers across the service area as necessary.
  • Incident reporting & confidentiality: Follow protocols to ensure safety and HIPAA compliance.
  • Adherence: Follow policies, procedures, and the organization's Code of Conduct.
  • Additional duties: Perform other responsibilities as assigned.

Qualifications

  • A Bachelor’s degree or RN license with 2+ years of relevant experience, or a Master’s degree with 1+ year may qualify.
  • Fluent in Russian and understanding of community cultural traditions.
  • Strong integrity and a personal commitment to serving people with I/DD and their families.
  • Excellent interpersonal skills and dedication to compassionate care.
