  • Salary Range: $29.87 - $31.93 per hour 💵

Position Summary

The Care Manager provides crucial services within our Care Management programs, including:

  • Health Home Care Comprehensive Care Management
  • HCBS Basic Plan Support
  • State Paid Care Management services

This position may support Willowbrook Class Members, ensuring they receive the personalized care they deserve.

Key Responsibilities

The Core Role

The Care Manager's primary responsibility is to oversee and coordinate access to essential services for individuals with intellectual and developmental disabilities.

The Care Process

Working closely with the member, their family or representative, and providers, the Care Manager develops, implements, and monitors an integrated, person-centered Life Plan. This process begins with a comprehensive assessment.

The Life Plan

  • Foundation: The basis for all service delivery
  • Goals: Address medical, behavioral health, community, social supports, and other services to promote health and fulfillment
  • Advocacy: Support members in accessing necessary services to achieve their goals and live meaningful, high-quality lives
