Position Summary

The Care Manager plays a vital role in providing services across our Care Management programs, including Health Home Care Comprehensive Care Management, HCBS Basic Plan Support, and State Paid Care Management services. This position may support Willowbrook Class Members and requires a compassionate approach to coordinate access to essential services.

Key Responsibilities

  • Deliver person-centered care: Ensure compliance with regulatory standards and agency policies while providing tailored care management services.
  • Assess and develop plans: Complete comprehensive assessments, identify service gaps, and create individualized Life Plans working with interdisciplinary teams.
  • Advocate for members: Support members in accessing services, benefits (Medicaid, Social Security, SNAP), and maintaining eligibility.
  • Coordinate healthcare: Facilitate access to medical, behavioral health, and specialized services, including transitional care and discharge planning.
  • Maintain documentation: Accurately complete all required service records and billing documentation within specified timeframes.
  • Utilize health IT: Maintain electronic health records and support telehealth services.
  • Travel as needed: Meet members and providers across the service area, ensuring their needs are met efficiently.
  • Ensure confidentiality: Follow HIPAA and agency privacy policies strictly.
  • Participate in team activities: Attend meetings, trainings, and supervisions; perform other related duties as required.

Qualifications

  • Bachelor's degree (B.A./B.S.), Licensed Registered Nurse, or Master's degree with relevant experience.
  • At least two years of relevant experience (or one year for Master's degree holders).
  • Strong understanding of community norms, culture, and traditions of Members and families.
  • High integrity with a personal commitment to serving people with I/DD and their families.
  • Excellent interpersonal, communication, and teamwork skills.
  • Ability to work independently and professionally in a respectful manner.

Work Environment

This position is not remote but offers a flexible combination of work-from-home, field visits, and in-office engagements. Employees must adhere to privacy standards and leverage secure WiFi connections and private spaces for sensitive conversations.

Travel throughout the service region is required, including driving or using public transportation to meet members and providers.

Physical Requirements

Roles involve verbal and written communication, operation of office equipment, travel between locations, and physical activities such as standing, walking, reaching, and lifting objects up to 25 pounds. Reasonable accommodations are available per applicable policies.
