We are seeking a dedicated bilingual professional to coordinate and manage patient care in a vibrant community health setting in New York, NY.
🌟 Responsibilities:
- Identify the targeted risk population based on PCP referral, risk stratification, and patient lists
- Handle case management assignments and review case progress according to center guidelines
- Coordinate patient care through ongoing collaboration with PCPs, patients/families, communities, and healthcare team members, fostering a team approach that includes patient and family as active participants
- Create and regularly update personalized healthcare plans by assessing patient wellness, needs, preferences, and care plan utilization
- Communicate effectively with the care team to establish and maintain productive working relationships
- Advocate for and collaborate with patients and their families to accomplish treatment goals
- Educate patients and caregivers about healthcare options and care plans
- Conduct regular follow-ups to evaluate progress, ensure care continuity, and promote health outcomes
- Summarize reports related to care management activities
- Maintain accurate documentation in electronic health records
- Establish connections to external social services, specialist providers, and payer care management services
- Participate in clinical service team meetings and activities
- Update knowledge base relevant to our service population
- Perform additional duties as assigned
🎓 Minimum Qualifications:
- Bachelor’s degree (preferably in healthcare-related field)
- Bilingual proficiency in English and Chinese (Mandarin and Cantonese)
- Availability to work on weekends is preferred
- Experience in a healthcare setting is a plus
- Self-motivated and able to work well in a team environment
- Strong interpersonal and <>communication skills (both written and verbal)
- Basic computer literacy
💼 Compensation & Benefits:
- Salary: $40,000 – $46,000 per year
