We are seeking a dedicated bilingual professional to coordinate and manage patient care in a vibrant community health setting in New York, NY.

🌟 Responsibilities:

  • Identify the targeted risk population based on PCP referral, risk stratification, and patient lists
  • Handle case management assignments and review case progress according to center guidelines
  • Coordinate patient care through ongoing collaboration with PCPs, patients/families, communities, and healthcare team members, fostering a team approach that includes patient and family as active participants
  • Create and regularly update personalized healthcare plans by assessing patient wellness, needs, preferences, and care plan utilization
  • Communicate effectively with the care team to establish and maintain productive working relationships
  • Advocate for and collaborate with patients and their families to accomplish treatment goals
  • Educate patients and caregivers about healthcare options and care plans
  • Conduct regular follow-ups to evaluate progress, ensure care continuity, and promote health outcomes
  • Summarize reports related to care management activities
  • Maintain accurate documentation in electronic health records
  • Establish connections to external social services, specialist providers, and payer care management services
  • Participate in clinical service team meetings and activities
  • Update knowledge base relevant to our service population
  • Perform additional duties as assigned

🎓 Minimum Qualifications:

  • Bachelor’s degree (preferably in healthcare-related field)
  • Bilingual proficiency in English and Chinese (Mandarin and Cantonese)
  • Availability to work on weekends is preferred
  • Experience in a healthcare setting is a plus
  • Self-motivated and able to work well in a team environment
  • Strong interpersonal and <>communication skills (both written and verbal)
  • Basic computer literacy

💼 Compensation & Benefits:

  • Salary: $40,000 – $46,000 per year
