Position Overview
The Care Coordinator is a vital member of TCC’s care coordination team, providing direct, compassionate support to LGBTQIA+ clients in areas such as mental health, housing, healthcare, and basic needs. This role emphasizes relationship-building, advocacy, and practical problem solving to ensure every client is met with respect, affirmation, and a clear path forward.
The Care Coordinator collaborates closely with clients, staff, and community partners to deliver responsive, trauma-informed, and client-centered care. This position is instrumental in advancing TCC’s mission to ensure that LGBTQIA+ individuals have access to the resources needed to thrive and succeed. 🌟
Areas of Responsibility
🔹 Direct Client Support and Case Management
- Provide individualized care coordination, including intake, assessment, goal setting, and ongoing follow-up
- Develop and implement client-centered service plans that reflect each individual's needs, strengths, and priorities
- Support clients in accessing mental health, housing, healthcare, and basic needs services
- Maintain consistent, trust-based communication to foster engagement and continuity of care
- Respond to urgent needs with calmness, resilience, and solutions-focused approaches
🔹 Resource Navigation and Advocacy
- Connect clients with local and regional service providers, ensuring warm handoffs when possible
- Advocate for equitable and affirming access across systems, especially for clients facing systemic barriers
- Stay informed about community resources and their eligibility criteria
- Guide clients through complex systems like benefits, healthcare, and housing processes
🔹 Documentation, Data, and Coordination
- Maintain accurate, timely, and compliant case documentation
- Track client outcomes, referrals, and service utilization for program evaluation
- Collaborate with internal teams to provide holistic and coordinated care
- Support data-driven decision-making and continuous improvement initiatives
🔹 Community Engagement and Organizational Support
- Assist with outreach efforts to promote awareness and accessibility of services, including tabling and community events
- Participate in TCC’s annual Pride in the Park, supporting planning and execution as needed
- Attend occasional evening and weekend events; additional hours are eligible for compensatory time
- Represent TCC positively within community spaces, embodying its mission and values

Qualifications
- Bilingual in English and Spanish preferred
- Bachelor’s degree in social work, counseling, public health, or related fields (or equivalent experience)
- 1-3+ years of experience in case management, care coordination, or social services
- Experience working with LGBTQIA+ communities
- Strong commitment to TCC’s mission and values
