Position Overview: Care Coordinator II (Bilingual Vietnamese)
The Care Coordinator II plays a critical role in supporting older adults, individuals with disabilities, and their caregivers to meet their goals for community participation and independent living. From initial assessments to ongoing care planning, you'll work closely with clients and their networks to navigate complex health and social service systems.
This role involves working within various programs such as the Family Caregiver Support Program, Medicaid Alternative Care, and Tailored Support for Older Adults, adapting to different eligibility policies.
Key Responsibilities
- Manage a caseload: Serve at least 35 active clients monthly and 80-90 distinct clients annually.
- Assessments: Conduct initial and ongoing client-centered evaluations aligned with program requirements.
- Client Visits: Meet clients at their homes or preferred locations.
- Care Planning: Develop, update, and document personalized care plans based on client goals.
- Resource Coordination: Identify and link clients to community resources and interventions promoting their well-being.
- Education & Empowerment: Provide information about healthcare systems and chronic disease management to foster confident self-care.
- Follow-up & Advocacy: Track referral outcomes, advocate for client needs, and ensure timely service connection.
- Partnerships: Build and maintain relationships with healthcare providers, community organizations, government agencies, and housing authorities.
- Team Collaboration: Participate in case reviews, training, supervision, and consultations.
- Record-Keeping: Maintain accurate, complete client records in line with program policies.
- Continuous Improvement: Engage in quality enhancement activities and observe client service patterns.
- Community Engagement: Help organize community events and represent Neighborhood House in networks and organizations.
- Team Spirit: Foster a cooperative, solutions-focused environment within the program and across departments.
- Additional Duties: Perform other responsibilities as assigned to support our mission.
