• Coordinate and schedule home health care services for clients
• Serve as the main point of contact for clients, families, and caregivers
• Communicate effectively in both English and Spanish
• Maintain accurate records and documentation of client information
• Assign caregivers and ensure appropriate staffing coverage
• Address client concerns and resolve service issues promptly
• Collaborate with healthcare providers to ensure continuity of care
