Job Overview

We are seeking a dynamic and energetic Bilingual Team Lead fluent in Russian and English to provide continuous support and communication between healthcare professionals, patients, and their families. Under the direction of a Licensed Clinician, this role involves scheduling visits, gathering patient information, executing care plans, building rapport, and supporting both patients and caregivers. Responsibilities

  • Welcome & Rapport Building: Initiate welcome calls to introduce the HNYMPC’s Program, build rapport, and inform patients/caregivers of the benefits of care management to ensure a positive experience 😊
  • Communication & Data Management: Explain coverage benefits, obtain and update accurate patient contact details, and maintain patient demographics.
  • Information Gathering: Telephonically complete questionnaires to gather essential information for timely referrals to medical, mental health, social workers, and pharmacists.
  • Provider & Appointment Coordination: Assist patients/caregivers in finding providers, scheduling appointments, and navigating provider communications (calling offices, faxing forms, requesting documentation).
  • Meetings & Follow-ups: Participate in assigned meetings (Regional, ICT, etc.) and follow up to ensure action items are completed.
  • Care Plan Support: Assist with execution of care plans, follow up with providers for prescriptions and DME, and ensure patient comfort with their equipment.
  • Educational & Legal Documents: Send health education materials, advance directives, and healthcare proxy forms.
  • Resource Provision & Issue Resolution: Address patient/caregiver questions, provide resources, transfer calls, and resolve issues promptly.
  • Transition of Care: Support clinician efforts by gathering information during hospital and SNF discharge calls, notifying PCPs, and coordinating necessary services.
  • Transportation & Follow-up: Assist with transportation arrangements and follow up after appointments to ensure continued care.
  • Confidentiality & Compliance: Maintain strict confidentiality adhering to HIPAA regulations.
  • Additional Duties: Perform other responsibilities as assigned to support team operations.

Qualifications

  • Education: High School Diploma required; Bachelor's degree preferred.
  • Languages: Fluency in Russian and English is REQUIRED with excellent communication skills in both.
  • Skills & Experience:
    • Excellent written and oral communication skills, knowledge of medical terminology.
    • 6 months to 1 year of telephonic experience with inbound & outbound calls; experience with health plans or physician offices preferred.
    • Ability to prioritize, plan, and handle multiple tasks simultaneously.
    • Reliable team player with attention to detail and flexibility.
    • Proficiency in Microsoft Office applications (Word, Excel).
  • Additional Knowledge: Knowledge of health plan benefits and guidelines is a plus.

Compensation & Benefits 💰

Annual Salary: $50,000.00 - $75,000.00
