Position Summary 📝

The Intensive Care Coordinator (ICC) delivers assessment, care planning, and service coordination activities for eligible clients. Working closely with health and social service providers, the ICC ensures that care needs are met holistically to stabilize participants, promote access to health and wellness, and reduce healthcare costs.

Responsibilities 🎯

  • Monitor Client Progress: Conduct ongoing assessments through face-to-face visits, escorts, written, electronic, and telephonic outreach activities, following guidelines from Health Home, DOH, OMH, and departmental policies.
  • Conduct Functional Assessments: Complete comprehensive, client-centered evaluations to identify medical, behavioral, and social needs and goals.
  • Schedule & Documentation: Develop and adhere to daily appointment schedules; inform supervisors of conflicts; maintain accurate records; participate in supervision sessions.
  • Develop Care Plans: Create, review, and update person-centered care plans based on assessment outcomes, in collaboration with clients and network partners, ensuring policy compliance.
  • Manage Caseloads: Maintain an accurate caseload by promptly responding to care level changes, including discharges and transfers.
  • Build Collaborative Relationships: Share updates and participate in case conferences with service providers and network partners to monitor health and care levels.
  • Address Crises: Respond to crisis situations swiftly, coordinating with supervisory staff, service networks, and legal entities as needed.
  • Deliver Direct Services: Assist clients with referrals, manage entitlements (Medicaid, SNAP, SSI), secure stable housing, and coordinate transportation to support wellness and healthcare compliance.
  • Record Keeping: Use Electronic Health/Medical Record systems and NADAP databases to document contacts and treatment details within specified timeframes.
  • Other Duties: Perform additional responsibilities as assigned.

Qualifications 🎓

  • Educational Requirement: Bachelor’s Degree in Social Work, Human Services, or related field required.
  • Preferred Experience: Degree in social work, public health, or mental health counseling with two years of experience, or a master’s degree with one year of experience. Standards may vary based on governing entities like DOH or OMH.
  • Knowledge & Skills: Advanced knowledge of professional practices, policies, and procedures; ability to determine appropriate courses of action.
  • Caseload Flexibility: Willingness to adapt caseloads based on client needs and acuity.
  • Bilingual Ability: Spanish language proficiency required.

Compensation 💰

Salary: $57,000 per year
