Position: Social Worker II (Bilingual / Spanish)
The Social Worker II plays a crucial role on the social service team, working under the supervision of the Clinical Coordinator or Director of Clinical and Social Services, and collaborating closely with the Building Manager.
Key Qualifications
- Master's Degree required; MSW preferred
- Minimum of two years of social work experience in human services
- Fluency in written and spoken Spanish and English
- Experience working with older adults and individuals with mental illness and/or substance use issues is a plus
Core Responsibilities 💼
- Assist residents in accessing and maintaining medical, mental health, and community services
- Help residents obtain and sustain entitlements and home care support
- Coordinate care between community service providers and facilitate socialization activities through referrals
- Support residents with budgeting, income management, and timely rent payments
- Follow up during hospitalizations, short-term rehabilitation, and out-of-facility stays
- Oversee discharge planning, including completing necessary documentation and applications
- Provide individual counseling and crisis intervention as needed
- Escort residents to appointments and facilitate group activities (recreational, educational, clinical)
- Participate in the intake process for new residents and maintain professional boundaries
- Work collaboratively with team members, sharing resident progress and needs
- Complete incident reports and adhere to confidentiality policies
- Promote diversity, equity, and inclusion in service delivery
- Maintain case records, support plans, and psychosocial histories accurately and timely
- Implement emergency procedures when necessary
- Attend supervision, team meetings, and staff trainings
- Perform other duties as assigned
Work Environment & Schedule 🕒
Standard work hours are from Monday to Friday, 9:00 AM – 5:00 PM.
Compensation 💰
This position offers a competitive yearly salary, commensurate with experience and qualifications.
