The Role
We are seeking Bilingual Care Coordinators to support patients with chronic conditions through Chronic Care Management (CCM). This is a phone-based, productivity-driven position where you'll primarily communicate with patients, coordinate care, and document interactions.
This role is perfect for clinical professionals comfortable handling high call volumes, structured schedules, and meeting daily productivity targets in a remote setting.
Work Schedule & Expectations 🕒
- Full-time, 40 hours per week
- Coverage required Monday–Saturday, 8:00 AM–6:00 PM in your support time zone
- Fixed schedule (no alternate shifts available)
- Spend approximately 7–8 hours daily on calls
- Meet productivity and quality metrics that are monitored regularly
What You'll Do
- Conduct monthly CCM phone calls with assigned patients
- Make 30–50 outbound calls per day 📞
- Complete all required patient interactions to achieve CCM service goals
- Educate patients about chronic conditions and care plans
- Coordinate with provider offices, pharmacies, and community resources
- Identify care gaps and social determinants of health
- Create and update personalized care plans in the EMR
- Accurately document all patient interactions
- Maintain HIPAA compliance and uphold patient confidentiality 🔒
Requirements ✅
- 1–3 years of experience as a CMA/RMA, LPN, or RN
- Active certification or multistate license for LPN/RN
- Strong verbal and written communication skills in English
- Comfortable on the phone most of the day
- Experience with EMR/EHR systems
- Highly organized, reliable, and capable of working independently remotely
Preferred Skills 🌟
- Geriatric patient experience
- Care coordination or case management background
- Strong verbal and written communication skills in Spanish
