📝 Responsibilities

What You'll Do:

  • Monitor patient care plans, ensuring compliance with medical regulations and home health standards. 🏥
  • Coordinate patient admissions, discharges, and transfers, ensuring all documentation and medical records are complete and up-to-date. 📄
  • Work closely with field nurses, aides, and staff to assign cases, manage schedules, and oversee timely patient visits. ⏱️
  • Provide education to patients and families regarding home health services, treatment plans, and medication management. 📚
  • Assist in developing and implementing quality control procedures to ensure top-quality patient care and regulatory compliance. ✅
  • Collaborate with billing to review patient records, ensure correct coding, and resolve billing or insurance issues. 💰
  • Maintain accurate patient files, clinical documentation, and compliance reports per regulations. 📁
  • Help develop and update policies and procedures to reflect best practices in home health care. 📌
  • Serve as the primary contact for patient and family concerns, addressing inquiries and resolving issues promptly. 📞
  • Assist in training new clinical staff and provide ongoing education on documentation, protocols, and regulatory updates. 🎓

🎯 Qualifications

What You'll Bring:

  • Professional, ethical, and respectful conduct. 🤝
  • Ability to provide personal care for non-ambulatory clients, use a Hoyer lift, maneuver wheelchairs, and transport clients to social activities. 🏃‍♀️
  • Strong communication skills — both oral and written; excellent interpersonal skills. 💬
  • Problem-solving skills with good judgment to anticipate and resolve issues. 🧠

Education and Experience:

  • Current RN license required.
  • Current CPR certification. ❤️
  • At least one year of nursing experience, preferably in home health or clinical administration. 🩺
