We provide comprehensive wraparound clinical services, upskill Community-Based Organization (CBO) staff to become Community Health Workers, and utilize our proprietary, data-driven platform, Arc, for seamless care coordination. About The Opportunity 🚀
We are forming a passionate team to revolutionize primary care for underserved populations. We seek a dedicated Full-Time Referral Coordinator to serve as a vital link between patients, providers, plans, and internal teams. Your role will involve managing the referral process, ensuring timely communication, and advocating for patient care continuity by navigating insurance and healthcare requirements.
This position is remote, but candidates must reside in the United States and be available to work PST hours.
What You’ll Do 🎯
- Process, track, and coordinate patient referrals to ensure timely appointments
- Schedule appointments with primary care providers and specialists
- Coordinate with pharmacies regarding medication receipt and pickup
- Verify patient PCP and insurance details, including prior authorization requirements
- Manage the Release of Information
- Assist with follow-up and information collection for non-urgent patient needs
What You’ll Need ✅
- At least 1+ years of healthcare operations or administrative support experience in a fast-paced environment
- Confidence to own your role and propose solutions for success
- Bilingual in English and Spanish
- Proven results-oriented attitude and track record of exceeding performance metrics
- Reliability and adaptability to improve care for underserved communities
- Strong technical skills, comfortable with multiple platforms (Google Suite, Slack, Looker, etc.)
- Quiet, HIPAA-compliant, internet-connected workspace
- Passion for supporting and enhancing care for underserved populations
