About the Role

As a Patient Intake Coordinator, you will serve as the essential link between clients and our healthcare services, ensuring a seamless onboarding experience for each participant.

Responsibilities

• Explain program benefits and conduct initial eligibility reviews (Medicaid and Managed Long-Term Care) for home care applicants.
• Make outbound calls to an assigned census of 40-50 active patient cases and answer incoming calls with a target of converting 30+ monthly conversations into qualified leads.
• Coordinate and manage the intake process, including gathering necessary documentation and information.
• Collaborate with healthcare professionals, caregivers, and clients to ensure smooth onboarding and enrollment procedures.
• Maintain accurate and confidential records of all intake activities and participant information.
• Provide exceptional customer service by educating clients and caregivers on program guidelines and addressing inquiries and concerns throughout the process. 😊

Requirements

• Fluency in both English and Spanish (written and verbal) is essential.
• Experience in a similar role in Intake or Onboarding for home care clients.
• Strong organizational skills with attention to detail.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively within a team.
• Proficiency in basic computer applications (Salesforce a plus) and data entry.
