Position Overview
Location: Rockland County, NY
Type: Part-time / Full-time
This vital role involves coordinating services, communicating effectively with diverse families, and ensuring compliance with New York State regulations to enhance the early intervention experience for children and their families.
Responsibilities & Duties
- 📝 Serve as the primary contact for assigned Early Intervention cases, supporting families throughout their journey
- 💬 Communicate effectively in both English and Spanish with families, providers, and agencies
- 🔧 Coordinate and monitor services according to Individualized Family Service Plans (IFSPs)
- 🤝 Collaborate with the NYS Department of Health, service providers, and internal staff
- 🗃️ Ensure timely, accurate documentation including case notes and service updates
- 🔒 Maintain confidential client records in compliance with HIPAA and federal/state laws
- 📄 Distribute and track documentation to families, providers, and regulatory bodies
- 🎓 Provide families with resources, education, and ongoing support for navigating services
- ⏱️ Monitor case progress to ensure timely service delivery
- 🤗 Deliver professional, empathetic, and culturally sensitive customer service
Required Qualifications
- 🔑 Bilingual fluency in Spanish and English

Experience/Education Pathways:
- Minimum of 2 years in a regulated service coordination role
- OR 1 year of service coordination + 1 year working with infants/toddlers with developmental delays
- OR 1 year of service coordination + associate’s degree in health or human services
- OR Bachelor’s degree in a health or human services field
