About the Role

As an Advanced Practice Provider—either a Nurse Practitioner or Physician Assistant—you will deliver high-quality, in-home care to patients with various chronic conditions, including:

• Diabetes
• Hypertension
• Chronic Kidney Disease
• Heart Failure
• COPD
• Depression
• Other complex health needs

You will collaborate within an integrated multidisciplinary team (Clinical, Behavioral Health, Pharmacy, Administrative) to establish lasting patient relationships and ensure continuity of care — in homes and via telehealth. This role offers the chance to be part of a patient-centered, value-based care model focusing on population health outcomes.

What We Offer: Compensation & Incentives 💰

• Competitive salary: $122,000 - $203,000 annually
• Skill development bonuses: $2,500 for each core and specialty competency completed
• Anniversary bonus: $5,000 annually

Key Responsibilities

• Provide evidence-based, patient-centric care that reduces hospitalizations and enhances quality of life in patients’ homes
• Conduct thorough assessments: evaluations, wellness exams, follow-ups, post-discharge visits
• Educate patients and caregivers on disease management, medications, preventive care, and testing
• Participate in virtual weekly care team meetings
• Coordinate care with primary care physicians, specialists, and ancillary providers
• Ensure accurate and timely documentation
• May require periodic travel to patient homes

Qualifications & Requirements 🎓

• Bilingual proficiency: Spanish, Chinese, Russian, Yiddish, or Bengali preferred
• Licenses: Active, unrestricted RN and NP licenses or Physician Assistant license
• Certification: Board-certified (NP: ANCC/AANP | PA: NCCPA)
• DEA Certificate: Current/unrestricted or eligible to obtain
• Experience: Minimum 2 years of direct patient care experience
• Travel & Transportation: Reliable vehicle, valid driver’s license, auto insurance
• Certifications: BLS certification (supported during onboarding)
• Skills: Initiative, independent critical thinking, decision making
• Compliance: Knowledge of HIPAA, OSHA, and other regulations
• Preferred: Experience with annual wellness visits, EHR systems (Athena, Salesforce), managed care, population health
