About the Opportunity
Pair Team is hiring a Bilingual Family Nurse Practitioner to support the launch of our Medicare ACCESS program across multiple states. This 100% remote role involves providing virtual care to Medicare patients with complex, overlapping medical, behavioral health, and social needs.
You will be part of a centralized, multi-state clinical team, collaborating closely with RNs, Lead Care Managers, and clinical leadership to:
- Conduct patient assessments
- Guide care plans
- Support escalations
- Help develop a scalable care model for older adults
Candidates must be comfortable working across EST and PST time zones as needed to effectively support patients and teams.
Key Responsibilities
- Provide virtual clinical assessments and care for Medicare patients across ACCESS markets
- Support initial and ongoing patient assessments, care planning, and clinical oversight
- Manage chronic and acute conditions, medication questions, and routine follow-ups via telemedicine
- Collaborate with RNs, Community Health Workers, and clinical leadership to close care gaps and ensure timely follow-up
- Offer clinical guidance during case reviews, escalations, and complex patient situations
- Support patients with conditions common in Medicare populations, including hypertension, diabetes, CKD, cardiovascular disease, chronic pain, depression, and anxiety
- Build trusted relationships with patients who may have limited access to consistent care
- Work with operations and product teams to improve workflows, documentation, and patient engagement
- Maintain accurate, timely, and compliant clinical documentation
What You'll Need
- 2+ active Nurse Practitioner licenses in California, Nevada, Pennsylvania, New York, Maryland, Georgia, West Virginia, Texas, Florida, or North Carolina
- Willingness to obtain additional licenses as we expand into new states
- Board certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner
- Fluency in both Spanish and English
- At least 2 years of clinical experience as a Nurse Practitioner
- Experience caring for Medicare, geriatric, or medically complex adult populations
- Comfort managing chronic conditions such as hypertension, diabetes, CKD, and cardiovascular diseases
