Job Summary
The Health Coach is a Medical Assistant who works under supervision from the Clinic Site RN Manager or Clinic Units Manager. As part of our chronic care management team, the Health Coach collaborates with patients and their healthcare providers to optimize care delivery. This role involves providing medical assistance, interpretation, coordinating patient care, and performing clerical tasks.
Essential Job Functions
Clinic Responsibilities
- Deliver high-quality Medical Assistant care adhering to clinic policies and promoting culturally competent services in line with the agency’s Guide to Service Excellence and Corporate Compliance Plan.
- Safeguard patient privacy rights and assist in resolving patient complaints.
- Participate in a year-long intensive training program to develop core competencies necessary for a Health Coach II position, performing all related duties under supervision initially, with the goal of working independently by the end of training.
- Take and record vital signs, recognize abnormalities, and report findings to nurses and providers.
- Interview patients to gather medical history using the SOAP format in preparation for visits.
- Obtain basic health information, relevant documents, and records.
- Participate in team huddles to prepare for patient visits and manage standing orders.
- Conduct basic in-clinic lab tests, process specimens, and perform tests such as ECG, spirometry, audiometry, and Snellen vision tests.
- Perform nursing procedures including immunizations, tuberculin skin tests, phlebotomy, and dressing changes.
- Complete forms for ancillary services or referrals as directed by physicians.
- Optimize patient flow and perform sterilization, infection control, and cleaning of clinic areas and equipment.
- Maintain clinic supplies and ensure examination rooms and nursing areas are well-stocked.
- Handle interpretation duties and make simple referrals as needed.
- Enter data and generate reports from the CDMS system.
- Collaborate with provider staff on patient care elements and execute orders as directed.
- Update electronic health records (EHR), ensuring accurate documentation, records retrieval, staff communication, and data reporting.
Technical & Ethical Responsibilities
- Adhere to policies and procedures, ensuring patient rights to privacy, safety, and confidentiality.
- Attend in-service education to enhance technical skills and knowledge.
Leadership & Teamwork
- Participate in staff meetings and subcommittees, provide input, and contribute to organizational goals.
- Identify and collaborate on solutions to work area problems.
- Demonstrate effective communication skills with patients.
- Assist in orientation and training of volunteers and new team members.
Minimum Qualifications
- Proven experience as a clinical Medical Assistant with proficiency in providing culturally compassionate care.
- At least one year of clinical experience in a healthcare setting.
- Bilingual/bicultural in an Asian language required (Cantonese and/or Mandarin).
Preferred Qualifications
- Commitment to serving the Asian community and familiarity with community health clinics.
- CPR certification required.
- Medical Assistant certification preferred.
Compensation & Benefits 💼
Pay Range:
$31.37 - $32.67 per hour
