Job Summary

As a Bilingual LCSW fluent in Russian, you will identify social, behavioral, and economic barriers impacting patient outcomes. Your role will involve clinical counseling, assessments, crisis intervention, and connecting patients with community resources. Collaborate with healthcare teams, patients, families, and community organizations to foster comprehensive, culturally responsive care.

Key Responsibilities

  • Conduct psychosocial and SDOH assessments: Evaluate barriers related to housing, food insecurity, transportation, financial hardship, medication access, social isolation, and healthcare services.
  • Develop care plans: Create individualized, patient-centered strategies addressing identified social and behavioral needs.
  • Facilitate referrals: Connect patients with community agencies, social services, behavioral health providers, and support programs.
  • Care coordination: Provide ongoing follow-up to ensure successful engagement with resources and services.
  • Assist with applications: Support patients in applying for Medicaid, SNAP, HEAP, housing resources, and other programs.
  • Collaborate with healthcare providers: Work alongside physicians, nurses, care coordinators, and health plan teams for integrated care.
  • Participate in interdisciplinary meetings: Join care team meetings, case conferences, and utilization discussions.
  • Support care transitions: Assist post-hospital discharge care and follow-up.
  • Patient education: Inform about community resources, healthcare navigation, and self-management strategies.
  • Engagement activities: Conduct outreach calls and follow-up to monitor ongoing social needs.
  • Address health gaps: Help close preventive and quality gaps by mitigating non-clinical barriers.
  • Advocate: Ensure patient access to healthcare and community support.
  • Maintain current knowledge: Stay updated on local, state, and federal programs and resources.
  • Documentation: Accurately record interactions, interventions, referrals, and outcomes in EMR systems.
  • Compliance: Ensure all documentation and workflows adhere to organizational and regulatory standards.
  • Escalation: Identify and refer patients needing behavioral health, crisis, or additional clinical support.
  • Support population health initiatives: Contribute to programs aimed at improving outcomes for underserved groups.
  • Tracking & reporting: Monitor patient outcomes, referral completion, and barriers impacting care management.
  • Quality improvement: Participate in audits, training, and departmental initiatives.
  • Patient education: Guide patients in chronic disease management, screenings, and medication adherence.
  • Build relationships: Maintain collaborative ties with community organizations and healthcare providers.
  • Maintain confidentiality: Protect patient information per HIPAA and organizational policies.
  • Additional duties: Perform special projects and other responsibilities as assigned.

Qualifications

  • Licensed in NY: Valid LCSW license in New York State
  • Language skills: Fluent in Russian and English
  • Experience: Proven background in motivational interviewing, family counseling, and working with children or disabled individuals
  • Medical knowledge: Familiarity with ICD and CPT coding, mental health diagnoses, and medical documentation standards
  • Technical skills: EMR systems experience; clinical research knowledge is a plus
  • Clinical skills: Crisis management, assessment, behavioral interventions
  • Work environment: Hospital or community care experience preferred
  • Sensitivity & professionalism: Handling PTSD, addiction, autism spectrum, geriatrics, or Alzheimer's with compassion

Compensation & Benefits

Salary: $90,000.00 – $120,000.00 per year 💰
