Job Summary

As an integral part of our team, you will identify and address various social, behavioral, and economic barriers that influence health outcomes and access to care. Your role involves providing clinical counseling, psychosocial assessments, crisis interventions, care coordination, and connecting patients with community resources—all within a culturally sensitive environment. Collaborate with patients, caregivers, clinicians, health plans, and community organizations to foster better health and social support outcomes.

Primary Responsibilities 📝

  • Conduct psychosocial and Social Determinants of Health (SDOH) assessments for assigned patients.
  • Identify barriers related to housing, food security, transportation, financial hardship, utilities, medication costs, social isolation, and healthcare access.
  • Develop patient-centered care plans tailored to social and behavioral needs.
  • Coordinate referrals to community organizations, social services, behavioral health providers, and support programs.
  • Provide ongoing care coordination and follow-up to ensure successful engagement with resources.
  • Assist patients with applications and enrollment for Medicaid, SNAP, HEAP, transportation, housing, and other programs.
  • Collaborate with healthcare and care team members to promote holistic patient care.
  • Participate in team meetings, case conferences, and utilization discussions.
  • Support transitions of care after hospital discharges, ER visits, rehab stays, or nursing facility admissions.
  • Educate patients and caregivers on community resources, healthcare navigation, and self-management.
  • Engage patients via outreach calls to assess ongoing social needs and barriers to care compliance.
  • Address non-clinical barriers to close preventive care and quality gaps.
  • Advocate for patients and families to ensure access to healthcare and community support.
  • Maintain updated knowledge of local, state, and federal social service resources.
  • Document interactions, interventions, referrals, and outcomes accurately in electronic systems.
  • Ensure compliance with organizational, CMS, Medicaid, Medicare, NCQA, and other regulations.
  • Identify patients who need escalation to behavioral health, crisis intervention, or adult protective services.
  • Support population health and health equity initiatives for vulnerable populations.
  • Track and report on patient outcomes, referral completion, and care barriers.
  • Participate in quality improvement, audits, training, and departmental meetings.
  • Assist with patient education on chronic disease management, screenings, medication adherence, and wellness programs.
  • Build and sustain collaborative relationships with community organizations and healthcare providers.
  • Protect patient confidentiality in accordance with HIPAA and policies.
  • Perform additional duties and special projects as assigned.

Qualifications & Skills 🎓

  • Valid licensure as a Licensed Clinical Social Worker (LCSW) in New York State.
  • Fluency in Russian and English is essential.
  • Experience in motivational interviewing, child & family counseling, or working with children/disabilities.
  • Knowledge of medical terminology, mental health diagnoses (ICD-10/ICD-9), CPT coding, and documentation standards.
  • Familiarity with EMR systems; experience in data collection or clinical research is a plus.
  • Strong crisis management, case management, assessment, and behavioral health skills.
  • Experience in hospital or community care settings is advantageous.
  • Ability to handle sensitive situations involving PTSD, addiction, autism, geriatrics, or Alzheimer's with professionalism and compassion.

Salary Range: $90,000.00 - $120,000.00 annually
