⭐ Duties

• Conduct thorough patient assessments, including vital signs, physical examinations, and health history reviews in both Spanish and English.
• Develop and implement individualized care plans aligned with patient needs and goals.
• Administer medications, injections, IV infusions, and perform wound suturing as needed following aseptic techniques.
• Provide specialized services such as dialysis support, ostomy care, tube feeding management, and ventilator assistance.
• Educate patients and families on disease management, medication administration, health coaching, and safety protocols.
• Document all patient interactions accurately using EMR (Electronic Medical Records) systems like Epic or Cerner while ensuring compliance with HIPAA regulations.
• Coordinate with physicians and interdisciplinary teams for discharge planning, case management, and urgent triage when necessary.

🎓 Qualifications

• Valid licensure as an RN (Registered Nurse), PT (Physical Therapist), MSW (Master of Social Work), or relevant healthcare professional with current state certification.
• Bilingual fluency in Spanish and English is essential for effective communication with diverse patient populations.
• Proven experience in home care settings or related fields such as long-term care, hospice & palliative medicine, or hospital medicine.
• Familiarity with EMR/EHR systems including Epic, Cerner, Athenahealth, or eClinicalWorks; proficiency in ICD coding (ICD-9/ICD-10) is a plus.
• Knowledge of medical terminology, physiology, diagnostic evaluation techniques, and medication administration protocols.
• Experience working with individuals with disabilities or developmental challenges is highly desirable.
• Ability to perform physical tasks such as lifting Hoyer lifts or assisting with mobility aids safely.
• Strong critical thinking skills for triage situations and acute pain management; experience in emergency medicine or trauma centers is advantageous.
