Job Summary

As a Coordinator at Elite Home Health Care, you will ensure the smooth and efficient management of home care services for our clients. Your responsibilities include overseeing an assigned caseload, coordinating service placements between clients and caregivers, and maintaining effective caseload management to guarantee timely and appropriate care. You will also collaborate with team members on staffing issues, update patient and caregiver records accurately, and assist with on-call duties as scheduled.

Your leadership, problem-solving, and communication skills will be essential in providing high-quality care and support to our patients and their families. 💙

Essential Duties and Responsibilities

  • Manage your assigned caseload to ensure clients receive optimal home care services.
  • Coordinate service placements between clients and caregivers using HHA eXchange.
  • Maintain effective caseload management to deliver timely care.
  • Participate in departmental training and meetings to stay current on policies and best practices.
  • Assist the team lead with coverage and address staffing issues collaboratively.
  • Discuss schedules, availability, and attendance openly with department staff.
  • Update patient and caregiver records accurately in HHA eXchange.
  • Verify caregivers' attendance daily, prioritizing staffing needs for no-shows or call-outs based on patient requirements.
  • Handle calls, emails, or faxes from contractors and clients requesting information or reports, and discuss patient statuses with nurse care managers.
  • Address caregivers’ concerns regarding working conditions or policies, working with management to resolve issues in their best interests.
  • Assist with scheduled on-call duties.

Qualifications and Skills

  • Previous experience in healthcare or related fields preferred.
  • Proficient in Microsoft Office Suite, HHA Exchange, and internet navigation.
  • Strong leadership and problem-solving abilities.
  • Exceptional organizational skills and attention to detail.
  • Excellent communication skills — both oral and written — to effectively interact with employees, patients, and families.
  • Ability to identify problems, analyze data, and take appropriate actions.
  • Ability to multitask effectively in a fast-paced environment.
  • Flexible schedule to meet the department and patients’ needs.

Pay Range 💵

The hiring range for this position is $24.00 - $27.00/hour. Final compensation will depend on factors such as experience, skills, certifications, and location.
