About the Opportunity

We are forming a passionate team eager to revolutionize primary care for those who need it most. We are seeking a full-time Healthcare Call Center Representative, known internally as Lead Care Manager. This pivotal role involves outreach and engagement with individuals experiencing homelessness, serious mental illness, substance use disorder, or high medical needs.

This position is entirely remote within the United States, but candidates must be able to work Pacific Time hours consistently. We operate in a fast-paced, high-volume environment, so comfort working within a structured PST schedule is essential.

What You’ll Do

  • Manage an ongoing caseload using evidence-based approaches to promote engagement and health goal achievement.
  • Build trust using relationship-based strategies, recognizing the diverse backgrounds and experiences of members.
  • Conduct regular telephonic and SMS outreach for follow-up and support.
  • Assist members in identifying health and wellness goals and integrating these into shared care plans.
  • Support care team members with delegated tasks, including nurses, behavioral health specialists, and community engagement staff.
  • Collaborate on care issues through systematic reviews and consultations before clinical actions.
  • Meet monthly encounter metrics to ensure regulatory compliance.
  • Identify and address barriers to care participation.
  • Help individuals connect with community resources by scheduling appointments, managing referrals, and ensuring follow-ups.
  • Coordinate physical care appointments with providers, both internal and external.
  • Utilize digital platforms for collaboration and daily task management.

What You’ll Need

  • At least 1+ years of professional experience in case management, care coordination, or healthcare call centers.
  • Bilingual proficiency in English and Spanish.
  • Strong cultural fluency and trust-building skills with diverse populations.
  • High level of empathy and patience, especially when facing adversity.
  • Ability to work effectively within a multidisciplinary care team.
  • Excellent organizational skills and keen attention to detail.
  • Comfort with emerging technology, including Google Suite, Slack, CRM databases, Excel, Word, email, and video conferencing tools.
  • A reliable, HIPAA-compliant workspace with stable internet for documentation and communication.
