Position Summary 🌟

We are looking for a caring and motivated Field Representative fluent in both English and Spanish to join our outreach team in New York City. Your primary role will involve traveling to the homes of Spanish-speaking residents to assist with Medicaid enrollment, renewals, and related support services.

This is a fully field-based position requiring daily travel. A personal, reliable vehicle is essential, as public transportation alone will not suffice.

Your efforts will directly connect families with vital healthcare coverage.

Key Responsibilities 📝

• Conduct home visits to explain Medicaid benefits and assist with applications and renewals.
• Guide clients through forms, documentation, and eligibility processes.
• Build trust and rapport by offering culturally sensitive support in the client's preferred language.
• Coordinate with family members, caregivers, or social workers as needed.
• Maintain accurate records of visits and follow-up actions.
• Participate occasionally in outreach at local events or partner organizations to raise awareness.

Requirements ✅

• Fluent in Spanish and English (spoken and written)
• Valid driver’s license and a reliable personal vehicle
• Experience in community outreach, healthcare, public services, or case management
• Familiarity with Medicaid or other public health programs
• Strong communication and interpersonal skills with diverse populations
• Comfortable with extensive travel to client homes
• Self-motivated, organized, and able to work independently
