Job Overview

We are looking for a talented RN to perform patient visits, supervise home health aides, and ensure the delivery of high-quality care. The ideal candidate may be bilingual in English and Spanish/Creole/Cantonese/Mandarin, though this is not mandatory.

Key Responsibilities

• Demonstrate proficient skills to admit and reassess home health patients in accordance with physician orders
• Coordinate and supervise Home Health Aides (HHAs) in patient care delivery and discharge planning
• Initiate home care visits and regularly reassess patient needs and progress
• Develop and initiate the plan of care under doctor’s orders
• Observe, assess, and document patient progress accurately
• Notify physicians and healthcare team of any changes in patient conditions
• Educate patients and caregivers on disease management, medications, and individualized treatment plans, including in-home health care education

Qualifications & Educational Requirements

• Current NYS Registered Nurse License
• Must be vaccinated against COVID-19
• Ability to work positively and effectively with patients, families, and staff
• Excellent communication and time-management skills
• Ability to travel to multiple patient homes daily
• Bilingual in English & Spanish
