Position Details
- Language Required: Spanish-English
- Location: Montgomery, Anne Arundel, and Prince George's Counties
- Salary: $46,000 annually
- Schedule: Hybrid schedule available!

Key Responsibilities
- Assessments: Evaluate individuals and families on mental status, personality, behavioral responses, relationships, and interactions.
- Planning: Develop service or treatment plans based on family needs, offering practical suggestions for positive change.
- Interventions: Deliver activity and participation-based strategies developed from training, experience, and success stories.
- Coverage: Provide 24/7 support for assigned individuals and families.
- Closure & Aftercare: Create comprehensive closure and aftercare plans for each family or individual served.
- Community Engagement: Build strong relationships with referring agencies, guide families in their interactions, and provide community resource referrals.
- Documentation: Keep confidential, organized, and complete records; submit documentation accurately and promptly.
- Travel: Willingness to travel up to 1 hour for family services.
- Team Participation: Attend team meetings, support peer development, and contribute to team performance and training initiatives.
Qualifications
- Education: Bachelor's Degree in a related Human Service field required
- Experience: Minimum of one year in case management or behavioral health
- Language: Bilingual (Spanish-English) required
- Training: Specialized training required
- Skills: Self-motivated, detail-oriented, excellent communication and analytical abilities
- Licenses: Valid driver’s license, reliable vehicle, registration, and insurance
