Position Summary 🚀

We are seeking a dedicated supporter to help individuals achieve their desired lifestyles by fostering independence, productivity, and social/physical inclusion. Your responsibilities will include:

• Developing and assisting with person-centered plans
• Providing transportation support
• Supervising and implementing daily program activities and community experiences
• Completing daily and follow-up documentation

Working Days & Hours 🕗

• Schedule: Monday - Friday
• Hours: 8:00 AM to 4:00 PM (40 hrs/week)
• Location: Employment Services

What You'll Be Doing Day-to-Day 📝

• Preparation for Change: Planning, listening to individuals, teaching community success skills, and ensuring plans reflect their preferences.
• Community Integration & Participation: Facilitating community activities, implementing person-centered plans, teaching skills, and serving as a positive role model.
• Personal & Health Care: Assisting with daily living, supervising safety, administering medications, reporting incidents, and following safety protocols.
• Communication & Documentation: Maintaining accurate records, attending meetings and trainings, supporting policies, and developing goals.
• Managerial Duties: Supporting staffing, teaching employment skills, liaising with outside agencies, and advocating for individuals' rights.
• General Responsibilities: Submitting reports, managing documentation, addressing urgent issues, and ensuring compliance with all policies.

You Are the Ideal Candidate If: 🎯

• Possess a Bachelor’s Degree or a high school diploma with relevant supervisory experience
• Are computer literate with excellent communication skills
• Dependable, reliable, with strong problem-solving abilities
• Hold current certifications and meet core competency requirements
• Can be an approved driver with reliable transportation
• Work well with individuals and team members, demonstrating excellent interpersonal skills
