Responsibilities

What You'll Do:

  • 📝 Maintain comprehensive records of assigned clients’ files and related paperwork.
  • 🔒 Ensure confidentiality of patient/client and agency information, adhering to HIPAA guidelines.
  • 🩺 Assign homecare aides based on client needs and the Plan of Care.
  • 🧑‍⚕️ Provide coaching to caregivers to uphold high-quality client care and escalate performance concerns when necessary.
  • 🔄 Communicate schedules and updates proactively with clients, aides, referral sources, and management.
  • 🏠 Conduct in-home visits to assess needs, maintain connection, and adapt care plans accordingly.
  • 📊 Prepare and submit routine departmental reports.
  • 📝 Document client conditions and overall service accurately and timely.
  • 📝 Record and review client satisfaction surveys and in-home visits to foster strong relationships and address concerns.
  • 🚩 Handle client complaints professionally, document issues, and report significant problems for resolution. (Note: This is a mandatory reporter role for critical incidents.)
  • 🎓 Educate and coach clients on changes to their Plan of Care.
  • 🤝 Maintain positive relationships with clients, aides, and referral sources.
  • 📋 Ensure compliance with all applicable laws, regulations, and company policies.
  • 🔧 Perform other duties as assigned.

For roles involving Developmental Disabilities (DD), compliance with all relevant state regulations on training, documentation, and practices is required. This includes in-person meetings, record-keeping, training completion, and adherence to state guidelines to provide effective and responsible support.

Education and Experience

  • 🧓 At least 18 years of age.
  • 🎓 High school diploma or equivalent preferred; higher education or certifications may be required.
  • 🩺 Current PPD or chest x-ray if applicable.
  • 💊 Experience with Medicaid, Waivers, or Home Healthcare preferred.
  • ⚠️ Other requirements in compliance with state or local regulations.

Management Authority

  • 🎯 Conducts performance reviews.
  • 🎓 Trains other team members.
  • 📝 Oversees and directs the work of associates.

Physical Requirements

  • 🔹 Ability to move, transport, or lift up to 50 pounds.
  • 🔹 Ability to traverse office and client home environments, including stairs.
  • 🗣️ Effective communication skills for clear information exchange.

Travel Requirements

🚗 Regular travel is expected daily or weekly, including in inclement weather, to serve clients effectively.
