Digital Health Information is an innovative system providing specialized care in the Long-Term Care community, focusing on patients living in their own homes. We are an independently owned LTC pharmacy, telehealth physician practice (not open to the general public), and remote patient care facility. Currently operating in Pennsylvania and New Jersey, we are expanding nationwide, one state at a time.

Position Overview 🎯

We are seeking a Spanish-speaking Certified Medical Assistant (CMA) to join our clinical care navigator team. This role involves managing essential front and back-office activities, ensuring seamless communication between physicians and our Spanish-speaking patient community, and providing remote patient care.

Key Responsibilities 📝

• Perform patient outreach via phone to Spanish-speaking patients 📞
• Assist physicians during telehealth appointments and translate for patients 🩺🌎
• Monitor patient vitals regularly 💓
• Follow up weekly on patient care progress 🗓️
• Contact patients during times of concern or need 📲
• Support Care Navigators with patient intakes and home assessments 🏠
• Schedule device deployment and installations based on patient diagnosis ⚙️
• Adhere to all regulations, including HIPAA compliance 🔒
• Perform other duties as assigned, based on business needs 🔄

Qualifications ✅

• Fluent in both Spanish and English (reading and writing) 🗣️✍️
• High School diploma or equivalent 🎓
• Certified Medical Assistant with 2+ years experience preferred 🩺
• Valid driver’s license and reliable transportation 🚗
• Experience with front-office (patient interaction) and back-office activities 🏢
• Experience in patient care (medication, vitals, etc.) 💊🩺
• Familiarity with Electronic Health Record systems (EHR), eClinicalWorks preferred 💻
• Strong attention to detail and accuracy 🎯
• Excellent customer service and communication skills 💬😊

Proposed Schedule 🕒

Monday to Friday, 9:00 AM – 5:00 PM (40 hours per week)

Compensation & Benefits 💰

• Hourly Rate: $20.00 - $24.00 (negotiable based on experience)
