About the Opportunity 🎯
As the Care Team Manager, you will be instrumental in our interdisciplinary care model—outreaching and engaging individuals with complex needs such as Serious Mental Illness, Substance Use Disorder, homelessness, or other high medical needs. Building trust and helping individuals access comprehensive, whole-health care is central to your role.
With lived experience and local community insights, you will collaborate closely with clinics, community organizations, and care coordinators. Leading six interdisciplinary pods, you'll support up to 18 Lead Care Managers (LCMs) to deliver stellar patient care and engagement for those with intricate medical and social needs.
This leadership position is perfect for a dynamic professional with a multidisciplinary care background eager to improve healthcare access and outcomes for underserved communities.
Note: Internally, this role is called Lead Care Manager, Team Manager.
What You'll Do 📝
- Lead and Develop Your Team: Foster trust and rapport with LCMs. Provide coaching, support, and management to ensure high performance and team cohesion.
- Ensure Quality of Care: Conduct bi-weekly audits of LCM work to maintain high standards—timely documentation, updated tasks, and personalized care plans.
- Monitor Performance Metrics: Use dashboards to track and improve key indicators. Handle escalations and troubleshooting effectively.
- Training and Onboarding: Develop and roll out training programs. Support hiring processes and maintain SOPs.
- Foster Collaboration: Act as the primary liaison among LCMs, clinics, community supports, and cross-functional teams to ensure seamless communication.
- Drive Process Improvement: Identify and communicate opportunities for enhancing care management and patient engagement efforts across teams.
What You'll Need ✅
- Minimum 2 years of managing large teams, preferably in healthcare or community care settings
- 2+ years engaging with community, patient navigation, or social work
- Experience working within multidisciplinary care models
- Bilingual in English and Spanish
- Strong cultural fluency and community resource knowledge
- Experience working with individuals facing complex needs such as homelessness or mental health/substance use disorders
- Technical skills with CRM databases, Excel, Word, email, and video calls
- A quiet, HIPAA-compliant home workspace with reliable internet and phone access
Preferred Qualifications 🌱
- Lived experience working closely with individuals facing complex needs
- 2+ years of case management experience
- Familiarity with motivational interviewing
- Knowledge of medical terminology
- Problem-solving mindset, resourcefulness, and a proactive attitude
- Highly organized self-starter who handles multitasking with ease
- Adaptability in dynamic environments
Compensation & Benefits 💼
- Salary: $70,000 - $80,000
