About the Role

We are a growing home care agency looking for an experienced Community Outreach Coordinator to strengthen our referral network and community engagement throughout New Jersey. If you're passionate about building partnerships, increasing referrals, and connecting families with vital in-home care services, this role is ideal for you!

Key Responsibilities

• Develop and maintain referral relationships with:
  • Hospitals, rehab centers, discharge planners, and social workers
  • Senior centers, adult day programs, and case managers
  • Cultural organizations, religious institutions, and community leaders
• Represent the agency at:
  • Health fairs, senior expos, and community events throughout NJ
• Drive client referrals and caregiver recruitment, especially bilingual Home Health Aides (HHAs) 🌐
• Educate families and community partners on home care services, Medicaid and private pay options, and how to access care 💡
• Serve as a cultural liaison to facilitate communication across diverse communities 🌎
• Track outreach activities, referral sources, and performance metrics 📊

Qualifications

• Experience in home care, healthcare outreach, or senior services

• Self-motivated with the ability to work independently in the field 📍
• Valid driver’s license and willingness to travel locally 🚗
• Education: High school diploma required; Associate’s or Bachelor’s degree preferred
