Overview 🩺

As a Bilingual Care Coordinator – LPN/LVN, you will support patients managing chronic health conditions through outreach, education, and care coordination. This role is perfect for bilingual nurses seeking to transition from bedside care into a telephonic environment while fostering meaningful patient relationships and promoting improved health outcomes.

You will serve as a trusted advocate, guiding patients through Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs, working closely with healthcare providers to ensure ongoing care and support.

🔥 Responsibilities

  • Conduct outbound patient outreach calls in both English and Spanish
  • Perform health risk assessments and support CCM and RPM activities
  • Accurately document all patient interactions within Electronic Health Record (EHR) systems
  • Educate patients and families on managing chronic diseases and ensuring treatment adherence
  • Identify care gaps and escalate concerns appropriately to care providers or teams
  • Coordinate care plans and maintain consistent follow-up schedules
  • Connect patients with community resources and healthcare support services
  • Uphold HIPAA compliance and confidentiality standards
  • Collaborate with providers and care teams to enhance patient engagement and outcomes

✅ Requirements

Must Have

  • Active, unencumbered LPN/LVN license (California or compact states accepted)
  • Residency within the MST or PST time zone
  • Professional fluency in both English and Spanish
  • Current Basic Life Support (BLS) certification
  • Strong proficiency with EHR systems and Google Suite
  • Dedicated, HIPAA-compliant home office with reliable high-speed internet and dual monitors
  • Excellent clinical reasoning, communication, and patient engagement skills

Nice to Have

  • Experience in care coordination, CCM, TCM, or remote patient monitoring programs
  • Background in patient and family education on chronic disease management
  • Familiarity with CMS standards, quality measures, and healthcare documentation
  • Experience assisting patients with community resources and support programs
