Position Overview

Full-Time | Location: Philadelphia, PA 19134 Area | Must Be Bilingual (Spanish Speaking)

Salary: $55,702.00

Schedule: Monday - Friday, 8:30 AM - 5:00 PM

Job Summary

We are seeking a Bilingual Blended Case Management Supervisor to lead and support our case management team. In this role, you will supervise Targeted Case Managers, provide guidance on service delivery, ensure compliance with program standards, and oversee the coordination of care for adults in the community. Your leadership will foster a collaborative team environment and help deliver high-quality, client-centered services.

Key Responsibilities

• Ensure adherence to: DBH Practice Guidelines within the scope of TCM duties to promote best practices.
• Supervision: Provide daily on-site supervision to Teams of Blended Case Managers through team meetings. Discuss daily contact plans, address team members' concerns, and facilitate collaboration for problem-solving and task coverage.
• Individual Support: Make every effort to provide weekly individual supervision to each assigned staff member, supporting their professional development and case management effectiveness.
