Job Summary

We are seeking a Bilingual Blended Case Management Supervisor to lead and support our case management team. In this role, you will supervise Targeted Case Managers, provide guidance on service delivery, ensure compliance with program standards, and oversee the coordination of care for adults in the community. You will play a key role in fostering a collaborative team environment and ensuring high-quality, client-centered services. 🤝

Key Responsibilities

• Implementation of Guidelines: Ensure that the DBH Practice Guidelines are effectively integrated into the general operations of the TCM duties.
• Supervision & Team Support: Provide daily on-site supervision to teams of Blended Case Managers, including leading team meetings to discuss daily contacts and cases.
• Collaborative Problem Solving: Use team members’ insights to address challenges and delegate tasks to ensure smooth workflow.
• Individual Supervision: Make every effort to provide weekly one-on-one supervision to each staff member, supporting their professional development and case management needs.
