Job Summary

We are seeking a Bilingual Blended Case Management Supervisor to lead and support our case management team. In this leadership role, you will:

• Supervise Targeted Case Managers
• Provide guidance on service delivery
• Ensure compliance with program standards
• Oversee coordination of care for adults in the community
• Foster a collaborative team environment
• Deliver high-quality, client-centered services

🔑 Key Responsibilities

• Implementation of DBH Practice Guidelines: Ensure that guidelines are integrated into the daily operations within the scope of TCM duties.
• On-site Supervision: Provide daily supervision for teams of Blended Case Managers through team meetings, discussing daily contacts, challenges, and support strategies.
• Team Collaboration: Utilize team members to address challenges, distribute tasks, and cover interactions as needed to promote efficiency.
• Individual Support: Make every effort to provide weekly one-on-one supervision for each staff member, supporting professional development and problem-solving.
