Schedule: Monday - Friday, 8:30 AM - 5:00 PM

Job Summary

We are seeking a Bilingual Blended Case Management Supervisor to lead and support our dedicated team. In this leadership role, you will oversee Targeted Case Managers, guide service delivery, ensure compliance with program standards, and coordinate community care for adults. You will foster teamwork and uphold high-quality, client-centered services. 🤝

Key Responsibilities

• Ensure Implementation of Practice Guidelines: Oversee the application of DBH Practice Guidelines within the daily operations of the Targeted Case Management (TCM) team.
• On-site Supervision: Provide daily supervision to teams of Blended Case Managers through team meetings, discussing daily contact plans, addressing challenges, and coordinating tasks.
• Team Collaboration: Utilize team members to solve problems and cover interactions or tasks that may require additional support, fostering a collaborative environment.
• Individual Supervision: Strive to provide weekly one-on-one supervision to each staff member to support their professional growth and address ongoing needs.
