About the Role

A Case Manager assesses, monitors, and advocates for clients and their families, helping them access community resources to enhance their quality of life and reduce hospitalizations. 💙

Key Responsibilities

• Create a Plan of Care tailored to the client’s needs
• Collaborate with the client's care team to ensure proper management of medical conditions
• Medication Management
• Link clients and families to crisis counseling, therapeutic services, and safety interventions
• Assist with applying/obtaining benefits such as HRA benefits, SNAP, WIC, Social Security, Food Pantries, Access-A-Ride, etc.
• Maintain weekly contact via telecommunications with clients and their families to address ongoing needs

Qualifications

• Bachelor’s Degree in Social Work, Psychology, Sociology, Human Services, or a related field (2+ years experience a plus)
• OR Master’s Degree in a related field (1+ year experience a plus)
• Spanish speaking (preferred)
• J1 visa holders are welcome to apply

Ideal Candidate Profile

Previous experience as a Case Manager, Adult Care Manager, Social Worker, or Care Manager is highly desirable.

• Type: Full-time and Part-time positions available
• Work Location: On the road (Hybrid model)
• Salary: $27,000.00 – $50,000.00 per year 💰
