Role Overview

The Behavioral Health Specialist at Oak Street Health plays a vital role in delivering care management and evidence-based short-term psychotherapy to patients with behavioral health needs. This key team member collaborates with Care Teams across primary clinics, supporting behavioral healthcare through assessment, referrals, and treatment planning. Based on performance, there is potential for remote work days.

Core Responsibilities 📝

  • Collaborate with primary care teams to identify patients suitable for the Oak Street Behavioral Health Program
  • Screen and assess patients for mental health and substance use disorders
  • Develop care plans and facilitate referrals to psychiatric consultants or external resources
  • Provide short-term counseling and evidence-based treatments
  • Coordinate care with psychiatric consultants, including warm handoffs for telehealth visits
  • Maintain accurate, timely documentation in electronic medical records
  • Participate in regional and organizational workshops, feedback sessions, and surveys to advance the program
  • Potentially cover multiple clinics based on clinic maturity and panel sizes
  • Perform other duties as assigned

What Are We Looking For? 🤔

Required

  • Master’s Degree in mental health counseling, social work, counseling, marriage and family therapy, or related fields from an accredited institution
  • Highest clinical level license (or local equivalent)
  • Experience screening for mental health and substance use disorders
  • Experience with assessment and treatment planning for mental health/substance use issues
  • Knowledge of brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Cognitive Behavioral Techniques)
  • U.S. work authorization

Strongly Preferred

  • Proficiency in structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Cognitive Behavioral)
  • Knowledge of differential diagnoses of mental health and substance use disorders

Preferred

  • Proficient in PC skills

Position Details

Weekly Hours: 40

Type: Full-time

Pay Range: $72,627.00 — $155,538.00 annually

This range reflects the base salary and may vary depending on experience, education, location, and other factors. The position is eligible for additional bonuses, commissions, or incentives in addition to the base pay.
