Overview

The Nurse Practitioner provides clinical leadership to promote compliance with quality and cost standards of care. They manage service delivery for inter-professional and para-professional teams on individual or group cases, functioning as a vital resource for clinical and operational guidance. This role involves delivering advanced nursing care aligned with current regulations and standards, working under the supervision of the Clinical Director for the Nurse Practitioner Program. Home visits are conducted in Manhattan.

What You Will Do

• Manage and deliver comprehensive advanced nursing practice for targeted patient populations, evaluating responses to therapy and updating care plans to ensure quality outcomes.
• Develop and update clinical protocols in collaboration with physicians and team members, providing necessary training.
• Provide holistic nursing care including physical exams, history taking, screening, emergent interventions, pharmacological and non-pharmacological treatments, ordering treatments, health maintenance, care management, and patient education. Establish and adjust treatment plans based on clinical findings.
• Collaborate across the care team with patients, families, primary care physicians, and other providers to assess and plan ongoing care, across different settings.
• Ensure compliance with Patient-Centered Medical Home standards, meaningful medical record data use, HEDIS, and QARR quality metrics.

Qualifications

Licenses and Certifications

• 📝 Current license and registration as a Registered Professional Nurse in New York State
• 🩺 Certified Nurse Practitioner in NY with a specialty in adult health, family health, or gerontology
• 🚗 Valid driver’s license (operational/regional needs dependent)
• ✅ Credentialed status with VNS Health Medical Care at Home and related managed care plans
• 📇 NPI, Medicaid, and Medicare provider numbers (preferred)
• 🤝 Collaborative practice agreement with a physician (preferred)
• 🔖 Certification by ANCC or similar accrediting body (required for billing and credentialing)
• 🔹 For Psychiatric Nurse Practitioners only: Current PMHNP-BC certification

Education

• 🎓 Master’s Degree in Nursing or equivalent from an accredited nurse practitioner program (required)
• 🧠 Current ANCC or AANP certification as adult, family, or geriatric nurse practitioner (required)
• 🔹 For Psychiatric Nurse Practitioners only: Master’s in psychiatric-mental health Nurse Practitioner (required); PhD preferred

Work Experience

• 👩‍⚕️ Minimum of two years practicing as a nurse practitioner utilizing full scope of practice (preferred)
• 🏡 Clinical home care experience or managerial experience (preferred)
• 📊 Knowledge of HEDIS and QARR quality measures, ICD-10, CPT coding for reimbursement
• 🗣️ Bilingual skills as operational needs dictate

Pay Range

USD $109,900.00 - USD $146,500.00 per year 💰
