📝 What You Will Do

• Manage and deliver full-scope advanced nursing practice tailored to specific patient populations, evaluating responses and updating plans for optimal outcomes 🩺
• Identify needs for new or revised clinical protocols; collaborate with physicians for development and training 🤝
• Provide comprehensive care including physical exams, history-taking, screening, emergent interventions, medication management, preventative care, referrals, discharge planning, counseling, and education 📋
• Develop treatment plans based on clinical findings; escalate evaluations when necessary 🔍
• Collaborate with patients, families, and healthcare teams for holistic assessment and care planning 💬
• Ensure clinical services meet standards of Patient-Centered Medical Home, meaningful use, and quality measures like HEDIS and QARR ✅

🎓 Qualifications

🔖 Licenses and Certifications

• Current Registered Nurse license in New York State 📝
• Licensed and certified Nurse Practitioner in NY, with a focus in adult, family, or gerontology health 🏥
• Valid driver’s license (may be required based on operational needs) 🚗
• Credentialed status with VNS Health Medical Care at Home and associated plans ✅
• Maintain NPI, Medicaid, and Medicare provider numbers (preferred) 📈
• Collaborative practice agreement with a physician (preferred) 🤝
• Certification by ANCC or equivalent accrediting body to bill Medicare and meet credentialing standards 🏅

🔖 Specialized Certifications (for Psychiatric Nurse Practitioners)

• PMHNP-BC certification required 🧠

🎓 Education

• Master's Degree in Nursing or related graduate program registered by NYSED for NP qualification 🎓
• Current ANCC or AANP certification in adult, family, or geriatric nurse practitioner 🩺
• For Psychiatric Nurse Practitioners: Master’s in psychiatric-mental health NP; PhD preferred 🎓

💼 Work Experience

• At least two years as a nurse practitioner with full-scope practice (preferred) 🏥
• Home care or managerial experience (preferred) 👩‍⚕️👨‍⚕️
• Knowledge of HEDIS, QARR, ICD-10, CPT coding for reimbursement 📊
• Bilingual skills based on operational needs 🌐

💰 Pay Range

USD $109,900.00 – USD $146,500.00 per year
