Overview

We provide clinical leadership to promote increased compliance with quality and cost standards of care. Our team manages service delivery across inter-professional and para-professional staff, focusing on both individual cases and populations. As a key resource, we offer guidance and support to achieve team outcomes, delivering advanced nursing care in accordance with State and Federal regulations and national standards. This role operates under the supervision of the Clinical Director for the Nurse Practitioner Program.

What You Will Do

• Manage and provide full scope of advanced nursing practice for targeted patient populations, evaluating responses to therapy and revising care plans as needed.
• Develop and update clinical protocols in collaboration with physicians and staff, providing training when appropriate.
• Deliver comprehensive advanced nursing care including:
  • Physical examinations and comprehensive histories
  • Screenings for physical and psychological conditions
  • Emergent interventions and pharmacological/non-pharmacological treatments
  • Ordering treatments and DME, preventative health activities
  • Care management, referrals, discharge planning, counseling, and patient education
• Coordinate with patients, families, and other healthcare providers to assess, plan, and provide continuous care management across various settings.
• Ensure clinical services comply with standards such as Patient-Centered Medical Home, meaningful use of medical records, HEDIS, and QARR measures.

Qualifications

Licenses and Certifications

• Current RN license and registration in New York State 📝
• License and registration as a Nurse Practitioner in New York, with a specialty in adult health, family health, or gerontology
• Possession of a valid driver's license (operational/regional needs dependent)
• Credentialed status with VNS Health Medical Care at Home and related managed care plans
• Maintaining NPI, Medicaid, and Medicare provider numbers (preferred)
• Collaborative practice agreement with a physician (preferred)
• Certification by ANCC or another accrediting body to bill Medicare and meet credentialing requirements 💳
• For Psychiatric Nurse Practitioners only: PMHNP-BC certification required

Education

• Master's Degree in Nursing or other qualifying graduate degree from an accredited nurse practitioner program 🎓
• Current ANCC or AANP certification as adult, family, or geriatric nurse practitioner
• Master's Degree in psychiatric-mental health Nurse Practitioner (required)
• PhD in psychiatric-mental health Nurse Practitioner (preferred)

Work Experience

• Minimum of two years’ experience as a nurse practitioner utilizing the full scope of practice (preferred)
• Experience in clinical home care or managerial roles (preferred)
• Knowledge of HEDIS, QARR, ICD-10, and CPT coding (required)
• Bilingual skills based on operational needs (required)

Pay Range 💰

USD $109,900.00 - USD $146,500.00 per year.
