🔍 Overview

As a Nurse Practitioner, you will offer clinical leadership to enhance compliance with quality and cost standards of care. You will manage service delivery among inter-professional and para-professional teams, serving as a key resource to guide clinical and operational activities. Your role includes delivering advanced nursing care aligned with state, federal, and national standards, working under the supervision of the Clinical Director for the Nurse Practitioner Program.

🩺 What You Will Do

• Manage and deliver full scope of advanced nursing practices for targeted patient populations
• Evaluate patient responses to therapies and adjust care plans accordingly
• Develop and revise clinical protocols in collaboration with physicians and practice staff
• Provide comprehensive care including physical exams, history intake, screenings, emergent interventions, pharmacological, and non-pharmacological treatments
• Manage preventative health activities, referrals, discharge planning, counseling, and patient education
• Establish treatment plans based on clinical findings and consult specialists or emergency care when necessary
• Collaborate with patients, families, and healthcare team members to deliver assessment and care planning
• Ensure compliance with Patient-Centered Medical Home standards, meaningful use of medical data, and quality measures like HEDIS and QARR

📝 Qualifications

Licenses and Certifications

• Current RN license and registration in New York State
• NY State Nurse Practitioner certification in adult health, family health, or gerontology
• Valid driver’s license (if required by operational needs)
• Credentialed status with VNS Health Medical Care at Home and managed care plans
• NP certification by ANCC or similar accrediting body (required for Medicare billing)
• Psychiatric Nurse Practitioners only: Current PMHNP-BC certification

Educational Qualifications

• Master's Degree of Science in Nursing or equivalent from an accredited nurse practitioner program registered by the NY State Education Department
• ANCC or AANP certification in adult, family, or geriatric nurse practitioner fields
• Psychiatric Nurse Practitioners only: Master's in psychiatric-mental health Nurse Practitioner; PhD preferred

Work Experience

• Minimum two years of nurse practitioner experience utilizing full scope of practice
• Clinical home care or managerial experience preferred
• Knowledge of HEDIS and QARR quality measures, ICD-10, CPT coding for reimbursement
• Bilingual skills based on operational needs

💵 Pay Range

USD $109,900.00 - USD $146,500.00 per year
